Plan Conditions
- Membership is valid for a one-year period from the date of purchase/enrollment. The annual membership fee is non-refundable and non-transferable. This is not an insurance plan and can only be used at Whitestone Family Dentistry.
- Members are responsible for scheduling and attending appointments within the 12-month membership period. Unused appointments are non-refundable.
- Discounts do not apply to dental services covered by insurance or billed to insurance programs. Insurance-covered services will have rates determined by the insurer or Whitestone Family Dentistry.
- Discounts under your membership do not apply to any treatment prior to joining.
Members must pay their bills in full at the time services are rendered to receive discounts.
Our Membership Cannot…
- Be used in conjunction with another dental plan or insurance
- Be used for hospitalization or hospital charges of any kind and cannot be used for services covered under workers’ compensation
- Be used for treatment outside the treating dentist’s capabilities
- Be used for referral to specialists
Whitestone Family Dentistry reserves the right to change covered services, terms and conditions, and the discounted fee schedule at its discretion. Membership fees can also be increased, but existing members will not be charged additional fees until their yearly renewal). Whitestone Family Dentistry reserves the right to terminate any member’s membership at its discretion.
- This agreement is governed by Texas law and is performable in Williamson County, Texas.
- This agreement supersedes all prior understandings or agreements between the parties. Any modifications must be in writing and signed by both parties.
- Any disputes arising out of this agreement shall be resolved by binding arbitration in Williamson County Texas, under the Federal Arbitration Act and the Commercial Arbitration Rules of the American Arbitration Association.
- In the event of a motion or litigation, the member is responsible for all costs, attorney fees, and interest at the rate of $300.00 per hour. By enrolling in the Dental Savers Membership, the member agrees to all terms and conditions herein.
